Financial Information
For your convenience, we accept cash, check, credit card and multiple payment plan options. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to any actual procedures.
Insured Patients
We are happy to provide you with a pre-surgical estimate of fees for your treatment plan. As a surgical specialist office, we deliver the finest care we are capable of at the most reasonable cost possible. In most cases, insurance does not cover the full cost of surgical care. It is your responsibility to understand your insurance policy and the coverage of benefits it provides. We will submit your insurance claim to your dental and medical insurance company as a courtesy to you. This does not imply or guarantee that your particular plan will cover your anticipated procedure either in part or in full. We require that you pay any deductibles, co-payments, and fees over your annual maximum at the time of surgery.
Payment Policies
Payment is due at the time of services, and any other arrangements must be made and approved before the day of your procedure.
